Podiatrist versus orthopedic surgeon? A board certified podiatric foot and ankle surgeon usually has much more experience in bunion surgery than the average orthopedic surgeon. An experienced, board certified surgeon is really the key, no matter what their credentials (DPM, MD, DO). Who should consider bunion surgery? If your foot hurts every day, in every pair of shoes, and you have failed conservative treatment which should include: wider shoes, anti inflammatories, padding, orthotics, and possibly steroid injection; you are a candidate for bunion surgery. Bunion surgery involves an incision along the top of the big toe joint and the removal and realignment of soft tissue and bone to restore normal joint alignment and to relieve pain. The first metatarsal bone is often cut, realigned and then stabilized with small screws. There are no guarantees that a bunion surgery will fully relieve your pain because of wear and tear arthritic change to the joint and nerve damage from the deformity. Most patients achieve at least 85% relief of their symptoms. Anesthesia selection is really patient and procedure specific, but light to moderate sedation, to make you sleepy, coupled with a local anesthetic block, similar to the dentist, is often used during the procedure. Some people do require general anesthesia due to a history of local anesthetic complications or other medical problems. The procedure usually takes a little more than an hour, depending on the type of surgery. A more complicated bunionectomy can take two hours or more. Then, the first metatarsal bone is cut in a V fashion then moved laterally to realign the joint. The cut or osteotomy is then stabilized with a pin or two small screws. This is the most common procedure and is known as an Austin bunionectomy. 3. Lapidus: Realignment of the soft tissue ligaments around the big toe joint. Removal of the part of the metatarsal head (the bump). Then, removal of a wedge of bone from the base of the first metatarsal and the bone adjacent to it (the cuneiform) as well as the cartilage surface of the two bones. The first metatarsal cuneiform joint is then aligned and stabilized with two large screws or a plate. This increases the stability of the area and decreases recurrence of a bunion deformity. This procedure is known as a Lapidus fusion and is usually performed in adolescents or adults with really flexible foot deformities. 4. Other Procedures: Fusion (arthrodesis) of the big toe joint or Total Joint Implant (arthroplasty) are also common, but usually used when the joint is severely damaged and is not repairable. The usual recovery period after bunion surgery is 8 weeks to 4 months, depending on the procedure and the health of the patient. Compliance also dictates how fast a patient heals. Swelling after surgery can last for up to a year. In a Keller or Austin, the patient is usually completely off their feet for just a few days, then in a walking cast or special shoe for 4 to 8 weeks. You can usually resume normal activity in 2 to 3 months. In a Lapidus fusion, most patients are in a hard below knee cast completely non weight bearing for 8 weeks, then a walking cast for 2 weeks, then a sneaker for another month. Normal activity is usually resumed in 4 months. Every patient is different. Patients with medical problems or osteopenia (soft bone) may require longer immobilization. Some patients require physical therapy after surgery. There are risks involved in having any type of surgery. No matter how good your surgeon is, if you do not follow directions you may have a bad outcome. Scarring, prolonged swelling, a stiff joint, numbness, shortening of the big toe, degenerative arthritis, infection and continued pain are the most common complications. More serious complications can include non healing of bone or a severe infection requiring a second surgery. After having bunion surgery, most people are happy with the results. A survey by the American College of Foot and Ankle Surgeons revealed 95% of patients with good to excellent outcomes form their bunion surgery. After having surgery, your ability to walk and be active is likely to improve. The big toe joint is usually much less painful and functions better. Even though their joint looks perfectly aligned and functions quite well, some peolpe are disappointed with their bunion surgery. This is usually due to unrealistic expectations. You will still not be able to wear extremely high heeled shoes after surgery and it is unrealistic to think that your joint will be "perfect" or function like it never had a problem. I explain how these topics tie together in a moment. Let start by defining Vision. To me, vision is the ability to know where you want to go and to see where you fit into the grand scheme of things. It your reason for being. It your life purpose. Once you have identified this, it becomes a type of compass for you. It keeps you focused in times of trouble and serves as your guide in making decisions about your future. Maybe your life purpose a simple one You want to earn a good living, raise happy children and retire healthy. Maybe, You want to have a fulfilling career, and be an integral part of your community. Maybe you want to make a difference in the lives of the elderly, or people with disabilities or abused or homeless people. To me, a personal vision is the big picture that ties together your Desires and enables you to focus on your goals. Personal Visions could focus on your own life or you ability to impact the world around you. They could cover one area or cover many areas. There are some prominent historical figures who had a Personal Vision that impacted the world. Abraham Lincoln vision was an end to slavery, and to promote equality in the human race. Gandhi vision was the independence of India from the British. Do you need a Personal Vision to have a successful happy life? No, you don Does a Personal Vision help focus you with everything that you do? Absolutely! One way to get you started with your Personal Vision is to answer this question? Imagine, just for a second, that you inherited a windfall of cash from a relative or won the lottery. Working to pay your bills was no longer your prime focus. What would you do with your life? Let me illustrate the importance of having a vision by telling you a little bit about my own personal vision. My personal vision is to continually grow mentally, physically and spiritually and to help provide information to other people so that they too can continue to growth mentally, physically and spiritually. My second personal vision it to keep re evaluating my personal vision and make sure it remains consistent with my thoughts and my feelings. A personal Vision can change as you change and grow as you grow. I started out this lesson with the importance of having a vision. The second thing I want to discuss is Visualizing or what some people call visualization. This is very different from Vision. The simplest description of visualizing is act of seeing your future success through positive mental images. want to share another quote with you. This one is from Earl Nightingale about visualization and goals. He suggests that you yourself in your mind eye as having already achieved this goal. See yourself doing the things you'll be doing when you've reached your goal. The clearer the picture, the better. Many people promote this technique, and studies in sports have demonstrated its success. For instance. at the height of his career the golfer Jack Nicklaus always attempted to "see" the successful drive or putt before attempting it. Earl method of visualization essentially asks you to start at the end. Begin with the end in mind. Let apply visualization to goal setting. For example, if you would like to buy a new car by the first of next month, you want to envision a mental picture of you driving that new car, or showing it off to your friends and family. By keeping that positive image in your mind eye, you effectively be pushing yourself even harder to fulfilling your vision. If you having trouble creating a clear mental image of your success, I like to share a couple of tips with you. Having something tangible to look at to help push you toward your vision can often help. For instance, let say you want a new car. Cut out a picture of the car you want, and paste it up in a prominent place where you can look at it whenever you need some motivation or stimulation. Try to remember this picture in your mind as often as you can on a daily basis. Remember that images are the language of the subconscious mind. Believe me visualization works. I like to leave you with another quote this one is one of my favorites, mainly because of its simplicity. It comes from Walt Disney, and all he said was you can dream it, you can do it. this point of the program, feel free to review any of our previous sessions. We talk again on Day 11. I find it so fascinating how limited we our with visions of what we would like to achieve in our lives, or what we want in our lives. A great way of bringing your visions to life is to create a bliss board. Find pictures in magazines of what you want to attract into your life. Choose images, words, captions and place them on the board and put the board somewhere you will see it every day. On my bliss board is a caption 'Educating mind, body and spirit' It inspires me everyday. It helps you believe that something is possible. But to achieve that goal still takes planning and time. The visualization is a a self motivational tool. Like for example say I am running a race on a hot sunny day. I get tired and want to quit. But then I visualize myself crossing that finish line. So instead of slowing down and giving up, I run towards the goal. I know that as long as I run along the marked path, I will reach the finish line. But if I quit or slow down or run through the grass, I may reach the finish line later or not at all. So in order to motivate myself, I have to see the finish line and know what steps to take to get there. I sure hate to be running in the wrong direction! Worst yet, running in circles!!!
